The 5-15 scoring system is a calculator, not a database. No personal information is stored. The professional submitting a questionnaire (only numbers, no text) receives a graph / table from www.5-15.org. (S)he is the only person who knows to whom these scores belong. This information of course has to be handled according to professional rules of confidentiality.
Electronic scoring is an option in 5-15R. The informant receives a code giving access to an empty questionnaire. When the questionnaire is filled in, the informant or anyone else cannot reopen it.
