Who is responsible for your information
| Information | Responsible |
|---|---|
| Questionnaire answers and results about a child | The registered clinician, or the organisation they work for, who carried out the assessment. Essence Instruments processes this information only on their behalf. |
| Clinician accounts, usage records and invoices, support messages and cookies | Essence Instruments. |
What 5-15.org does
The 5-15R questionnaire maps a child's strengths and difficulties and compares the answers with a large reference group of children and young people of the same age and sex. The results are shown only to the clinician who entered them. They are never used as the only basis for conclusions about a child.
Questionnaire information
- We hold only the questionnaire answers, as numbers, and the results calculated from them.
- They are stored without names, dates of birth or any other details that identify the child or the person who completed the form. We check the data we hold to make sure no identifying information is stored.
- Only the clinician who created the case can link the results to a child.
- Questionnaire information is deleted after a maximum of 12 months.
- We do not use it for our own purposes, research, statistics, marketing or invoicing, and we do not group it across clinicians without their specific authority.
Clinician accounts and invoicing
We hold the registration details you provide, your account records and the number of uses recorded against your account each month. From 1 September 2026, use is charged at £5.00 GBP per respondent. To invoice you, we use only your registered email address and the number of uses recorded against your account each month. No questionnaire content or respondent information is used for invoicing.

- Parents, carers and young people: please contact the clinician who carried out the assessment. They are the only people who can identify the child's record. If you contact us, we will pass your request to the clinician and help them respond, including with deletion.
- Registered clinicians: you can see all data held about your account and cases at any time by logging in to 5-15.org. You have the right to be removed from our register, and the questionnaires you created or entered will also be deleted. If you need a copy for your professional records, tell us first and we will provide it securely.
